This is for an eye exam at Costco. Remember that the Ophthalmologist is not a Costco employee, they bill separately.
I have a small charge showing up for which my insurance is demanding a receipt. I did not recognize it at first but I do recall now, that this doctor is the eye doctor at Costco and I paid my bill in portions because my benefit was maxed out and the small remainder went onto another card. This is a legit expense, not a scam. I just wish there was a more convenient way to get a receipt... some 2 years later.
